Targeted Case Manager

Description

This is a full time position that requires the ability to work a flexible schedule, general daytime hours Monday-Friday but must be available if emergencies arise or for house meetings in the evenings. Assist referral sources in identifying clients' needs; educate potential client and family regarding services provided, facilitate admissions process; communicate with appropriate referral source and to facilitate necessary admissions paperwork; maintain files to meet all internal, state, and federal quality and compliance standards; responsible for performing initial assessments and developing and implementing Service Plans; link clients to needed community resources and support services, such as house, employment, and financial and medical benefits - documentation is an essential part of the responsibilities; assure the goals, objective, and quality of service delivered to each client are fulfilled, provide case management service in accordance with written consumer-specific service plan which assesses strengths, needs, interests, and is goal and outcome oriented.

Required Knowledge, Skills, Abilities, Education, or Experience

4½ years experience in the field of developmental disabilities; OR bachelor's degree + 6 months experience in the field of human services; OR equivalent combination of education and experience.
