Night Ward Clerk

Summary of Position: This position supports patient care delivery by providing effective communication and customer service skills, documentation/organization of medical information and as representative of the department and the hospital to visitors, vendors and the public. This individual must be a patient advocate with the ability to multi-task, maintain information and the medical records in an orderly manner while prioritizing the needs of a busy department. Essential Functions and Responsibilities: 1. Supports Mission and vision of CBH. 2. Supports and adheres to Nursing Policies and Procedures. 3. Establishes and maintains a safe, organized, and orderly work environment by complying with procedures, rules and regulations and adhering to infection-control policies and procedures. 4. Participates in the orientation of new personnel, including 1st time temporary personnel. 5. Attends monthly Nursing staff meetings (50% required). 6. Maintains orderly patient charts. Reviews all patient charts each shift to purge duplicate documents. Ensures all needed forms are present by anticipating upcoming documentation needs. 7. Assembles new charts for admission, disassembles charts when patients are discharged. 8. Informs nurses of reports or requests from other departments. 9. Obtains past medical records, if requested. 10. At the beginning of each shift creates a status board for their shift and updates throughout the shift. 11. Completes and maintains each patient kardex by frequently reviewing the status board for new physician orders and updating the kardexs in a timely manner. 12. Compiles patient census data and quality data as assigned. 13. In the absence of an Admission Clerk, registers all patients into the computer using the meditech system. Demonstrates proficiency in both the short (quick) and long (full) registration. 14. Ensures patient records and charges are complete and prepared for HIM review. 15. Helps patients by responding to intercom requests; obtaining needed services from the nursing staff as indicated. 16. Possesses excellent customer service skills. Greets visitors, in person or on the telephone, answers inquiries, relaying calls, gives directions, refers inquiries to nursing and medical staff as indicated. 17. Reviews the staffing sheet for the following shift and the following day. 18. Maintain department office supply inventory by checking stock to determine inventory level, anticipating needed supplies, and placing order as needed. 19 Checks medical records for absence of physician signature and/or nurses signature. flags the records needing signatures. 20. Provides assistance to physicians and nursing staff in meeting quality measures. 21. Assembles physician charts for rounds. 22. Reads and records refrigerator/warmer temps on a daily basis. 23. Maintains the white board with accurate staff assignments and patient census. 24. Helps the nursing staff coordinate patient education, transfers and follow-up. 25. Helps answer patient call lights as time allows and at the Charge Nurses discretion. 26. Performs other duties as assigned.

Required Knowledge, Skills, Abilities, Education, or Experience

Skills and Qualifications: High school diploma or equivalent is required. CNA certification is required. The ward clerk must have sufficient knowledge of English, spelling and math to compile reports, make out requisitions, maintain simple records and perform various clerical tasks. The ward clerk must have knowledge of medical and technical terminology. The ward clerk should have good communication skills and mental and physical health sufficient to meet the demands of the position.
